Case Description:

San Jose, California – This is a 30 year old man with adolescent gynecomastia and presents to the San Francisco office for male breast reduction treatment. His goal is to get rid of his puffy nipples. He underwent gland excision and liposuction at Marin Cosmetic Surgery Center in Marin County, California. We removed 27 grams of breast tissue from the right chest and 21 grams of tissue from the left. He had an uneventful post op course and his pictures are seen four weeks after surgery.

HISTORY: This is a 32-year-old resident of Tracy, California who presents to the San Francisco, California office for evaluation for gynecomastia. He has adolescent gynecomastia that developed from approximately 12 to 13 years old. He does not take off his shirt and found information on gynecomastia on a TV show. This sparked his interest to pursue a surgical solution. His goal is to have a more natural looking chest. He underwent breast gland excision and chest liposuction with 150 cc of fat extracted from each chest. This was followed by excision of 18 grams of breast tissue from his right chest and 21 grams of tissue from his left breast. His procedure was performed at Marin Cosmetic Surgery Center in Marin County, California. His photographs were taken 3 months after surgery.

This 29-year-old man suffers from adolescent gynecomastia, and prior to surgery had never exposed his chest in public. He underwent glandular excision and suction-assisted lipectomy of the chest, from which I excised 23 grams of glandular tissue from the right and 21 grams from the left side.

HISTORY: This is a 25-year-old male resident of Berkley, California who presents to the San Francisco, California office for evaluation for gynecomastia. In addition he wishes liposuction of the abdomen and flanks. He has adolescent gynecomastia. He feels comfortable with his body and able to take off his shirt in public although a bit reserved. His goal is to have a “well defined chest with no excess fat”. He underwent gynecomastia breast gland excision and chest liposuction. He had ultrasound liposuction of the abdomen and flanks. He had 325 cc of fat extracted from his right flank and 375 extracted from his left. He underwent liposuction of his chest with extraction of 100 cc of fat from each side. He had 44 grams of tissue excised from his right breast and 24 grams excised from his left. His surgery was performed at Marin Cosmetic Surgery Center in Marin County, California. His photographs were taken three months after surgery.

This is a 30-year-old male from Sunnyvale, California, who has undergone testosterone replacement due to low testosterone. He developed an increase of fullness and thick in his chest after the hormonal therapy was given. He underwent gland excision and liposuction at Marin Cosmetic Surgery Center. He had 41 grams of tissue removed from his right chest and 34 grams removed from his left chest. His overall goal was to have a flattened chest with some minimal scarring. His photographs are seen approximately six months after surgery.

Note that the nipples are pointing downward in the pre-op photos.  It is difficult to reposition upward without an areola lifting procedure.

This is a 22-year-old male who had gynecomastia surgery performed in 12/2014 in which he had gland excision and liposuction, but was not happy with his surgical outcome. He presents to my office for revision gynecomastia surgery. He underwent gland excision, and suction-assisted lipectomy at Marin Cosmetic Surgery Center and his photographs were seen approximately four weeks after surgery. His overall goal was to achieve a flatter chest where the nipples would not show through a tight fitting shirt, and his nipples would be completely flat.

San Francisco, California – This is a 38 year old male who has undergone gynecomastia surgery twice in the past due to anabolic steroids. He has had gynecomastia surgery in 2003 on the right side only and in 2012 he had surgery on his left side. Since that time he has been on and off steroid therapy. His goal is to have a better contour of his chest. He is 5 feet 8 inches tall and weighs 190 pounds. His surgery was performed at Marin Cosmetic Surgery Center in Marin County, California. He had 30 grams of breast tissue and scar removed from his right chest and 47 grams of breast tissue removed from his left chest. He had a small amount of liposuction of 150 cc. on his left side and 125 cc. of fat and aspirate from his right side. His post op course was uncomplicated. His photographs are seen approximately two months after surgery.

This 29-year-old man is 5’10” and 184lbs. He suffers from adolescent gynecomastia, and prior to surgery had never exposed his chest due to embarrassment and uncomfortability. He underwent glandular excision and suction-assisted lipectomy, from which I removed a total of 325cc of fat from the right side of the chest and 400cc from the left. His goal was to “decrease nipple puffiness.”

This is a 23-year-old male from Napa, California, who presents with steroid induced gynecomastia and presents for gynecomastia treatment. He underwent gland excision and suction-assisted lipectomy at Marin Cosmetic Surgery Center in Marin, California. He underwent 48 grams of tissue removed from the right chest and 45 grams of tissue removed from his left chest. His photographs are seen at four months after surgery.

This 17-year-old young man is 6’2″ and 280lbs. He suffers from adolescent gynecomastia which has caused him extreme embarrassment. He underwent glandular excision and suction-assisted lipectomy of the chest and axillae, from which I removed a total of 1200cc of fat bilaterally, as well as 109 grams of glandular tissue from the right side of the chest and 840 grams from the left. Due to significant excess skin and stretched areolae, he will require a secondary procedure to tighten up the peri-areolar area

This is a 38 y.o. male 5′ 9″ and 193 lb who underwent male breast reduction surgery for adult gynecomastia. He is seen 1 month after surgery.

Note he is still in the early healing period.

Case Description:

This is a 63-year-old resident of New Hampshire who presented to the San Francisco office at Union Square for evaluation for gynecomastia. He developed his gynecomastia at a young age, but over the years it has become progressively worse. He is 5 feet 10 inches tall and weighs 155 pounds. He never takes off his shirt due to the extreme amount of embarrassment. He underwent gland excision with a superior incisional lift and liposuction of his chest. He had 6 centimeters of tissue removed and sent to Pathology from his right side and 6 centimeters from his left side. He had 150 cc of fat liposuctioned from each side. His surgical procedure was performed at Marin Cosmetic Surgery Center in Marin County, California. Note the overall improvement in his chest contour. This is a very complicated case. Due to his age being 63 years old, his skin is loose with less elasticity and contractility as well as sun damage to the area. This is, overall, a good outcome that can be improved by further peri-areolar lift for tightening of his skin.

This is a 25-year- old male from Geyserville, California, who presents with adolescent gynecomastia. He underwent gynecomastia surgery at Marin Cosmetic Surgery Center in which he had 71 grams of tissue removed from his right chest and 41 grams from his left chest. His postoperative pictures are seen approximately two months after surgery. His overall goal was to have a flat chest with a natural appearance.

San Francisco, California – This is a 54 year old male who is 5 feet 8 inches, 165 pounds, who had a 100 pound weight loss over 12-month. He presents to the office for evaluation and treatment of his male breast enlargement. He never takes off his shirt in public due to the size of his male breast and is uncomfortable wearing tight fitting clothing. His goal is to be able to take off his shirt in public and not be conscious of his appearance. He would like to wear tighter fitting garments. He underwent surgery at Marin Cosmetic Surgery Center in Marin, California. He had gland excision and liposuction of his chest as well as liposuction of his axilla and a superior crescent breast lifts. He had 99 grams of tissue removed from his right side and 96 grams of breast gland removed from his left side. He had an uneventful recovery and his photographs are seen approximately three months after surgery.

This is a 27-year-old male from Patterson, California, who had previous gynecomastia surgery from another plastic surgery practice. He was not happy with his outcome and felt that his right side was still too enlarged, and his left side was too puffy. His surgery was in 2007 and his goal at this time is to have a natural looking chest. He underwent gland excision and suction-assisted lipectomy in which he had 44 grams of tissue removed from his right side and 86 grams of tissue removed from his left side. He healed uneventfully, and his photographs are seen two months after surgery.

This 23-year-old man is 5’6″, 150lbs. Having dealt with gynecomastia since puberty, he sought out my help to “have a flat chest and feel comfortable taking off my shirt in front of others.” In hopes of achieving the best result, he worked hard to lose twenty pounds prior to surgery. To correct his gynecomastia, I performed suction-assisted lipectomy along with glandular excision of the chest.
