Case Description:

This is a 40-year-old male from Oakland, California, who presents for gynecomastia evaluation of the chest and liposuction of his abdomen and flanks. He underwent gland excision and suction-assisted lipectomy, as well as liposuction of the abdomen and flanks. He had 69 grams of tissue removed from his right chest and 89 grams of tissue removed from his left chest. His overall goal was to be comfortable in taking off his shirt in public. His photographs are seen approximately seven months after surgery.

Rohnert Park, California – This is a 24 year old male who presented to the Novato, California office for evaluation of his gynecomastia. He had breast enlargement since adolescence as well as having weight fluctuations as high as 300 pounds. His goal is to have a normal looking chest and to be able to wear tank tops and tight fitting shirts. He is 6 feet 2 inches tall and weighs 255 pounds. His surgery was performed at Marin Cosmetic Surgery Center in Marin County, California. He had 107 grams of breast tissue removed from his right chest and 101 grams of breast tissue removed from his left chest. In addition he had 750 cc. of fat and aspirate removed from both his right and left chest. He had an uneventful post op recovery. He underwent one steroid injection on his left side. This is a 22-year- old male from Elk Grove, California, who presents with adolescent gynecomastia and presents for gland excision and liposuction. His goal is to be able to take off his shirt. He wants a little more flat chest and a smaller areolar. He underwent gland excision and liposuction at Marin Cosmetic Center. He had 50 grams of tissue removed from his right breast and 52 from his left breast in addition to liposuctioned fat tissue. His photographs are seen approximately three months after surgery.

He underwent male breast gland excision and liposuction at Marin Cosmetic Surgery Center in Novato, California. He had approximately 175 cc of fat removed from each side. His pathology was approximately 18 grams of tissue on the right and 22 grams of tissue on his left. His postoperative photos are seen approximately two months after surgery.

He has male breast enlargement from a very early age of about 10 years old, therefore, adolescent gynecomastia. He does not take off his shirt in public due to the embarrassment. He underwent gland excision and suction assisted lipectomy at Marin Cosmetic Surgery Center in Marin County, California in which he had 250 cc of fat aspirated from each chest through a periareolar incision. This is a 24 y.o. male 5’9″ and 184 lb who underwent male breast reduction surgery for adolescent gynecomastia. He is seen 2 months after surgery.

Case Description:

This 38-year-old man is 5’10” and 155 pounds. He initially had gynecomastia surgery done twice between 1999 and 1991 by another surgeon, but was displeased with the outcome. I performed a revision procedure on him, consisting of glandular excision and ultrasonic liposuction of the chest and abdominal areas. I excised a total of 650cc of glandular tissue bilaterally form his chest. His goal was to achieve “total masculinity of the chest area.”

He had removal of 58 grams of breast tissue from his right side and 36 grams of breast tissue from his left side. In addition he had 300-400 cc. of fat and aspirate removed from each side of his chest. His post op course was uncomplicated. His photographs are seen three months after surgery.

San Francisco, California – This is a 34 year old male who presents with adolescent gynecomastia to the San Francisco office for gland excision and suction assisted lipectomy. He takes Propecia but has had no issues with enlargement of the chest after he started Propecia. He is 6 feet 1 inch, 170 pounds. His surgery was performed at Marin Cosmetic Surgery Center in Novato, California. He had 47 grams of breast tissue removed from his right chest and 43 grams of breast tissue removed from his left chest. In addition he had approximately 350 cc. of fat and aspirate removed from each side. His photographs are seen approximately four months after surgery.

He presents for breast reduction gland excision and suction assisted lipectomy. His goal is to be more comfortable and secure with himself. He is 5 feet 8 inches, 160 pounds. He underwent gland excision and suction assisted lipectomy at Marin Cosmetic Surgery Center in Novato, California. He had 35 grams of breast tissue removed from his right chest and 31 grams of breast tissue removed from his left chest. In addition he had approximately 150 cc. of fat and aspirate removed from each side. His post op course was uneventful. His photographs are taken approximately four months after surgery.

This is a 31-year-old male with grade 4 severe gynecomastia who presents for free nipple mastectomy of his gynecomastia tissue and fat. Due to the laxity of his skin and the amount of tissue it was elected to do a one stage procedure instead of a two stage gyno male breast reduction surgery. He underwent free nipple mastectomy with removal of a large amount of tissue on both his right and left side. We extracted about 1600 cc of fat and aspirate in addition from the abdomen and he also had flank liposuction. His photographs are seen approximately six weeks after surgery.

He had 54 grams of tissue removed from his right chest and 43 grams of tissue removed from his left chest. His photographs are seen three months after surgery.

San Jose, California – This is a very pleasant 24 year old man who presents with adolescent gynecomastia at the San Francisco office. He presents for male breast reduction evaluation. Dr Delgado’s plan is for gland excision and suction assisted lipectomy. He has had this condition since adolescence. He has had no history of steroid use and he does not take off his shirt in public. His goal is to have flat masculine chest. He underwent gland excision and suction assisted lipectomy at Marin Cosmetic Surgery Center in Marin County, California. He had 28 grams of tissues excised from his right side and 45 grams removed from his left breast. He had an uneventful recovery and his photographs are seen at about six weeks after surgery.

This is a 51 y.o. male 6’4″ and 240 lb who underwent male breast reduction surgery for adult gynecomastia. He is seen 5 months after surgery.

Note this is a difficult case because he is a better candidate for skin removal.  He did not want the larger incisions so a compromise was made in the final outcome.  His result is much improved but shows skin laxity on the left side.

Case Description:

Richmond, California – This is a 30 year old male who presents with adolescent gynecomastia and treatment with male breast reduction surgery. Due to the complexity of his case, it was suggested that he would probably require a secondary procedure which involved a periareolar lift for a more optimal outcome due to the laxity of his chest skin. He underwent gland excision and suction assisted lipectomy at Marin Cosmetic Surgery Center, followed by a Stage II periareolar lift for skin tightening. In the initial surgery he had 42 grams removed from his right side and 37 grams removed from his left side. His photographs are seen approximately 7 months after the initial surgery. His initial surgery was complicated by having a left chest hematoma which was evacuated the following day.

He is 6 feet 4 inches tall and weighs 195 pounds. He has adolescent gynecomastia. His goal is to have a reduction in the glandular tissue. He underwent gland excision and suction-assisted lipectomy of his chest at Marin Cosmetic Surgery Center in Marin County, California. He had 4 centimeters of tissue removed from his right side and 5 centimeters from his left. These photographs were taken six months after surgery.
